Add alt text option to 'insert image' pop-up

Summary:
Add another text box to the 'insert image' pop-up for posting entries which would allow the use of alt text for the image in question.

Description:
There is a tool on the HTML editor which writes your image HTML for you, if you insert the URL of the image. However, if you want to provide alt text, there's no way to do that using the tool, so you have to add the code by hand. This sort of defeats the purpose of having the tool in the first place.

It seems that it might be an easy improvement, and could offer a lot of benefit for users who want to provide image descriptions to screen readers (or commentary on the images, or in-jokes, etc).

In addition, the height and width should be added to this pop-up, so that people's images can be treated properly by the image placeholder preference.
